Professional Witness
Now and then, you may need a professional witness. A commissioner is a licensed person the Province of Ontario authorizes to take oaths or declarations for an affidavit or statutory declaration.
These are the types of documents you may need to be commissioned:
- Amendment to birth/death/marriage certificate
- Statutory Declaration of Common-law Union
- Statutory declaration for single status and proof of residency
- Affidavit for lost citizenship and immigration documents
- Statutory declaration of lost passport or permanent residence card (must include police report number)
- Delayed statement of live birth
- Proof of residency (owner/tenant letters)
- Legal name change forms
- Statutory declaration by a person for change of sex designation
- Canadian citizenship forms
- Statutory declaration for a statement of common-law status, supporting children, and marital status
- Affidavits
- Court Documents
- Pension Documents
- Sworn Statement for the Transfer of a Used Motor Vehicle in Ontario

Additional signatures are charged at CAD 14.95 per (2nd, 3rd, through the 10th signature).
Additional signatures are charged CAD 9.95 per (11th, 12th, and up).

Instructions
Before meeting with us for our Commissioner of Oaths services, please ensure you:
1. Have one government-issued photo- identification (e.g. passport, permanent resident card, driver’s license) or two government-issued non-photo identification (e.g. SIN card, birth certificate);
2. Have unsigned copies of the documents requiring signature;
3. For certified true copies – have the original documents as well as clear photocopies; and,
4. Have a witness available if your document requires a witness other than the Commissioner of Oath.
